Dermal fillers are a popular non-surgical treatment to restore volume and enhance facial features. However, the thought of needles and potential pain can be daunting for many. Here’s a detailed look into how you can reduce pain and have a more comfortable experience.

 

Understanding Pain Factors in Filler Injections

Dermal filler injections can be administered in various facial areas, including the cheeks, lips, under the eyes, and nasolabial folds. The level of pain can vary depending on the treatment area due to the density of nerve endings. For instance, areas like the nasolabial folds and marionette lines tend to be more painful compared to the cheeks and lips.

Topical Anesthetic Creams

One of the most common methods to reduce pain is the application of topical anesthetic creams. These creams are applied to the treatment area about 5-15 minutes before the procedure. They work by numbing the skin, making the injection process significantly less painful. 

 

I use compounded numbing creams, which can be formulated in higher concentrations for better efficacy.  For example, I typically apply 30% strength for my patients.  Prescription topical cream is usually only available at 4% or less, since it is not compounded and formulated for aesthetic injectable patients. 

 

Local Anesthetic Injections

In addition to topical creams, some HA fillers contain a local anesthetic like lidocaine, which further numbs the area after the initial injection. This dual approach ensures that the treatment area is adequately numbed, reducing pain sensations as the procedure progresses. 

 

Ice Packs and Cold Anesthesia

Applying ice packs before and after the injection can help numb the area and reduce swelling and bruising. Cold temperature works by constricting blood vessels and dulling nerve endings, which helps in minimizing pain during the procedure.

 

Vibration Anesthesia

Vibration anesthesia is an innovative technique where a handheld device is used near the injection site. Your body and nerves feel the vibration more than the actual injection.  Vibration competes with pain signals in the brain, effectively distracting the patient and reducing your perception of pain.

 

Studies have shown that vibration anesthesia can significantly decrease pain during facial injections, making it a preferred method for many practitioners.

 

Distraction Techniques

Simple distraction techniques such as tapping, massaging, or squeezing the skin near the injection site can also help. These methods work by diverting the brain’s attention away from the pain, making the injection process more tolerable.

 

Ultrafine Needles

Using ultrafine needles, such as 32 or 33-gauge needles, can minimize discomfort. These smaller needles cause less tissue trauma, leading to a less painful experience. Additionally, using multiple needles during a session ensures that each injection is sharp and precise, further reducing pain.

 

Nerve Blocks

For areas around the mouth, nerve blocks can be used to provide complete numbness for about 20 minutes. This method is particularly useful for patients with low pain tolerance or those undergoing extensive filler treatments.

 

Post-Injection Care

After the injection, applying ice and taking over-the-counter pain relievers like Tylenol can help manage any residual discomfort. It’s also important to follow aftercare instructions to minimize swelling and bruising, ensuring a smoother recovery process.
