6 Effective Ways to Fix Unwanted Fat After Facial Fat Grafting

Facial fat grafting is a popular cosmetic procedure aimed at enhancing facial volume and contour by transferring fat from other parts of the body to the face. However, not everyone is satisfied with the results, and some may find themselves with excess fat in certain areas. If you’re looking to reduce or eliminate this excess fat, there are several options to consider.

 

1. Massage and Manual Manipulation

One of the simplest methods to address excess fat post-grafting is through deep facial massage to the area of fat grafting. Gentle manipulation of the area can help redistribute the fat and improve the overall contour. While this method is non-invasive, it requires consistency and may not yield significant results for everyone.

 

2. Steroid Injections

Steroid injections, such as triamcinolone, can be used to reduce localized fat deposits. These injections work by breaking down the fat cells in the treated area. It’s essential to work with your plastic surgeon to determine the appropriate dosage and frequency, as overuse can lead to unwanted side effects.

 

3. Kybella or Deoxycholic Acid

Using Kybella, which contains deoxycholic acid, for off-label purposes has gained attention for its potential to address unwanted fat in areas beyond the submental region. While Kybella is FDA-approved for reducing fat under the chin, some practitioners have explored its use in dissolving fat grafts in the face. 

 

When injected, Kybella breaks down fat cell membranes, allowing the body to naturally eliminate the destroyed cells. This process can be applied to areas where excess fat from grafting has resulted in an undesirable appearance. 

 

However, it’s important to note that using Kybella in this manner is considered off-label and should be approached with caution. The procedure should only be performed by an experienced healthcare professional who can carefully assess the risks and benefits. 

 

Potential side effects, such as swelling, bruising, nerve injury, and other risks must be considered, and patients should have realistic expectations about the outcomes. 

 

4. Liposuction

For more significant fat reduction, liposuction may be an option. This procedure involves the surgical removal of excess fat and can be tailored to target specific areas. While effective, liposuction is invasive and requires downtime for recovery. Certain facial areas may not be easily able to undergo liposuction. 

 

5. Radiofrequency or Ultrasound Treatments

Non-surgical options like radiofrequency (RF) or ultrasound treatments can help reduce excess fat by using energy to heat and break down fat cells. These treatments are generally safe and require little to no downtime, making them an attractive option for those seeking non-invasive solutions. Typically, areas of fat grafting need to be very close to the skin surface for RF Microneedling to be the most effective. 

 

6. Surgical Excision

In some cases, revision surgery may be necessary to achieve the desired results. This could involve removing or repositioning the grafted fat. It’s important to consult with a skilled and experienced plastic surgeon to discuss the potential risks and outcomes of revision surgery.

 

7. Cryolipolysis (CoolSculpting) Is Not An Option

Cryolipolysis, commonly known as iCoolSculpting, is a non-invasive procedure that freezes fat cells, causing them to break down and be naturally eliminated by the body. While primarily used for body contouring, some practitioners offer CoolSculpting for facial areas, especially under the chin.

 

However, cryolipolysis is not the best option for removing unwanted fat grafting in the facial facial. It can not be safely applied in areas of the upper, mid, and lower face.  The amount of “melting” can not be easily modulated as well, making it a non option for revision of facial fat grafting procedures.
