Choosing the right size of breast implant is a critical task in breast implant surgery. I take utmost care with this before I carry out surgery. The best approach is to select implants that will look like a natural part of your body. Implants that are too large can leave a patient looking "busty." Most of the implants that are chosen by the patients are between 200cc and 550cc. The largest implant available legally today is 1200cc. When the implants settle, they usually look slightly smaller. Therefore, patients are encouraged to pick one size slightly larger than they like in the mirror (i.e., if you like 350, get 375). The base diameter of the breast is the key measurement determining the selection of implant size. Breast implants are designed in such a way that diameter (the foot print of the implant) and projection (the thickness, or the amount it sticks out away from your body) increase proportionately with the volume. Standard implants are designed so that most women who choose a given volume will be able to accommodate the breast implant's diameter beneath her breasts, because women tend naturally to choose an implant volume that is in proportion with body size and breast diameter. Most women (over 95%) are best suited to standard implants. If a woman with a smaller breast diameter desires a larger volume breast implant, placing a standard implant would be too wide for her body frame and would likely either touch in the middle of the touch, or drift too much into the underarm area. In such cases, I usually recommend a high profile implant with the same volume, but narrower diameter (footprint) and greater projection (the amount that it sticks out away from your body). An easy way to choose standard implants or a high profile is: First, your surgeon should measure your breast diameter. Next, you should choose the implant volume that you want. Finally, your surgeon should compare the diameter of the desired implant size (standard implant) to the measured diameter of your breast. If the diameter of the implant size you selected is about the same or smaller than your measured breast diameter, then a standard implant is appropriate. If the diameter of the desired breast implant is larger than you breast diameter, then you should consider thicker, or high- profile breast implants. The most commonly requested cup size in my patients is a C. However, for a petite woman or a larger woman, a full B cup or D cup, respectfully, may produce a more natural look. Sizers are good way of confirming that you are making the right implant choice. You can place the sizers in either a surgical bra or a sports bra. Dr. Roy Kim has sizers available in his office that are exact replicas of many different breast implant sizes and shapes. He recommends trying on the implants in front of a mirror with several different outfits. Sizers can help you visualize the post-operative size more clearly! Also, the breast implants are not directly related to cup size. There are no implants with instructions written on the box that translates to A, B, C or D cup. Implants can be chosen by the patients depending on their size, height, width, chest length, chest-torso width, as well as the amount of breast tissue, present or not, to achieve the cup size they wish long-term.
